Composition
Hydrocortisone, 0.5% eye ointment, each gram contains:
Active substance: hydrocortisone acetate - 5 mg;
Auxiliary substances: methylparaben - 2 mg;
Eye medical vaseline up to 1000 mg.
Pharmacological properties
Hydrocortisone - glucocorticosteroid. It has anti-inflammatory, anti-allergic, anti-itching and anti-swelling effects. Stabilizes cellular and subcellular membranes, including cellular and lysosomal membranes. It slows down the alteration phase and reduces inflammatory cell infiltrates, reduces the migration of leukocytes and lymphocytes in the area of inflammation. It reduces the binding of immunoglobulins to receptors on the surface of cells and inhibits the synthesis and release of cytokines (interleukins and interferon) from lymphocytes and macrophages. It reduces the release of arachidonic acid from phospholipids and the synthesis of its metabolites (prostoglandins, leukotrienes, thromboxane). Reduces the exudative reaction, helps to reduce capillary penetration.
Pharmacokinetics
Hydrocortisone poorly penetrates the cornea into the intraocular fluid. Hydrocortisone penetrates the epithelium and epidermis of the mucous membrane and is absorbed into the bloodstream in small amounts. Hydrocortisone absorbed from the mucous membrane is metabolized directly in the epithelium and epidermis of the mucous membrane, and then a small amount of it after absorption into the blood circulation is biotransformed in the liver. In the blood, 80% is bound to transcortin and 10% to albumin. It is excreted in the form of metabolites and in small amounts in unchanged form with urine and small amounts with bile. Absorption of hydrocortisone from the mucous membranes into the systemic circulation is more pronounced in children than in adults.

Method of administration and dosage:
A small amount of ointment is placed in the lower conjunctival sac 2-3 times a day for no more than 2 weeks. If necessary, after the clinical evaluation of the effectiveness of the therapy, the use of the drug can be continued. In post-traumatic conditions, the drug is prescribed 1-2 times no earlier than 7 days after injury or surgery.
Side effects
Local reactions: delay in the healing of defects of the corneal epithelium with the possibility of infecting a deeper part of the eye (in case of use during the violation of the integrity of the corneal epithelium); spread of infectious eye diseases, especially viral ones; It is possible to develop glaucoma (use for a period of more than 3-6 weeks, especially in prone patients), development of cataract (in case of repeated courses of treatment for many years).
Systemic reactions: in case of increasing the frequency of use of the drug in young children, it is possible to develop systemic reactions characteristic of glucocorticosteroids.
With long-term continuous use of hydrocortisone, it is possible to inhibit the function of the pituitary gland and the adrenal cortex.
Contraindications
- individual increased sensitivity to the components of the drug.
- Purulent, viral, tuberculous and fungal diseases of the eyes.
- Primary glaucoma.
- Corneal epithelial defects.
- Vaccination period.
- Children age up to 1 year.
Special instructions
Exceeding the course of treatment is not recommended. During the treatment, we should refrain from using contact lenses. In the case of using other eye drops, the time interval between their application and applying the ointment is at least 15 minutes. When using for more than 2 weeks and in the presence of open- and closed-angle glaucoma in the anamnesis, it is necessary to control the intraocular pressure. It is possible to use hydrocortisone for the treatment of children under the doctor's prescription if the expected therapeutic effect outweighs the risk of side effects. Children are more likely to develop systemic effects of hydrocortisone than adults. Therefore, the drug should be used in children with caution, and if possible, with a short course (5-7 days). Excessive use during the day, frequent use can cause hydrocortisone absorption and systemic action. The use of the drug is contraindicated in children under 1 year of age.
Effects on the ability to drive and operate machinery
The drug does not limit psychophysical activity, the ability to drive vehicles and operate moving mechanisms. For patients who temporarily lose visual acuity after the application, it is not recommended to drive a car or work with heavy machinery after the same application.
Pregnancy or breastfeeding
Use during pregnancy is possible in cases where the expected benefit of therapy for the mother exceeds the potential risk for the fetus. It should be used with caution and for a short time during breastfeeding.
Drug interactions
Drug interaction with other drugs practically does not exist.
Release form and packaging
3-3 g in an aluminum tube with a lid for medical ointments. The tube with the ointment and the leaflet for the use of the drug is placed in a cardboard box.
Storage conditions:
Store out of reach of children, in a cool and dry place at a temperature of 5º to 15ºC.
Shelf life - 2 years from the date of manufacture. The drug is not used after the expiration date.
